Employer Order Form

Employer Participation Agreement — Annual Participation in the Annum Housing Platform

This Order Form is entered into pursuant to the Master Services Agreement (MSA) between Annum Inc. and the Customer, and governs participation by the undersigned Employer in the Annum platform and associated housing programs.

Purpose & Employer details

Participation gives your workforce access to housing-related services, financial support programs, and marketplace solutions, while providing visibility into workforce housing needs, utilization, and impact.

Services provided

Subject to the terms of the MSA, Annum shall provide Employer with access to the following.

A. Employee Access to Platform

  • Access for employees to the Annum Platform
  • AI-assisted navigation of housing options, programs, and services
  • Personalized housing profile creation and ongoing updates
  • Eligibility screening and matching to available programs

B. Housing programs & benefits

Optional. Select the employer-sponsored housing programs and financial services you would like more information about.

A one-time integration/admin fee of 1.5% on the total fund amount applies to elected programs.

C. Employer Reporting

  • Employee participation and engagement metrics
  • Program utilization rates
  • Aggregated workforce housing needs insights
  • Estimated retention and recruitment impact indicators (if available)
  • Reporting delivered in aggregated and anonymized formats

D. Program Administration Support

  • Employee onboarding and engagement support
  • Program coordination with Customer and CBO partners
  • Ongoing customer support for Employer and employees

Pricing & term

Access to the Annum platform is provided at no cost to Employer.

Employer access fee

$0

No fee for platform access, onboarding, or reporting.

Optional Housing Programs & Benefits

1.5%

Of funding amount, agreed in writing via amendment or addendum.

Term. This Order Form is coterminous with the Customer's MSA term unless otherwise terminated in accordance with the MSA. Employer may opt into or discontinue participation subject to reasonable notice requirements defined by Annum program guidelines.

Data ownership & Employer responsibilities

How employee data is owned, used, shared, and protected under this Order Form.

A. Employer Data Rights

  • Employer retains ownership of all employee data directly contributed by Employer.

B. Platform Data Use

  • Program eligibility determination
  • Service delivery
  • Aggregated reporting and system optimization

C. Data Sharing

  • Data shared with Customer is aggregated and anonymized by default
  • De-identified to prevent attribution to specific individuals or Employer
  • Identifiable data requires explicit employee consent and/or written Employer authorization

D. Data Security & Retention

  • Commercially reasonable administrative, technical, and physical safeguards
  • Upon termination, data handling follows MSA retention and destruction provisions

Employer responsibilities

  • Support employee awareness and adoption of the platform
  • Comply with applicable data privacy and employment laws
  • Coordinate with Annum on program implementation as reasonably required

Optional charitable donations

Employer may elect, but is not obligated, to contribute financial resources supporting employee housing outcomes through Annum's Donor Advised Fund partner.

Acceptance & signature

In the event of conflict, the order of precedence is: this Employer Order Form, the Master Services Agreement, then any applicable Statements of Work or Amendments.

Your typed name constitutes a legally binding electronic signature.

Annum, Inc. will countersign upon receipt and return an executed copy to the primary contact listed above.
